Please print both the "Parent " and "Teacher" forms below for the appropriate visit "Initial" or "Follow-Up".

Parent or guardian completes the "Parent" form.

Current school teacher completes the "Teacher" form.

To decrease your waiting time, please drop off both forms (Parent and Teacher) at the office a week before the scheduled appointment.

If unable to bring in early, please bring both forms (Parent and Teacher) with you to your child's appointment.
